i've followed directions on advancing the appliance two turns every 3 days...i'm still not at the "sweet spot" and my tmj on my left side is absoluetly killing me.. how should i go about this? moving it back ?

remember I am NOT a doctor.....

I would take it slower than 3mm if thats causing a problem. Definitely go back to a lower setting. I went a week or more between settings, especially when I started to get out aways.

Are you doing your jaw exercises EVERY morning? And "working" the jaw during the day is not a bad idea, especially initially.

Take some anti-inflamitories (e.g. Alleve) to get it back under control. Also try some "anti- sensitive" rated toothpaste, especially at first.

Those are my SUGGESTIONS, from my NOT A DENTIST OR DOCTOR perspective.

I tried appliance and didn't help me more jaw pain and apnea got worse pressure got increased in cpap. You cannot gain weight otherwise you need to pull your appliance for another centimeter.

Yes, I would pull back on advancing. It is amazing how I wear my TAP II all the way up to the max and don't feel a thing. When my dentist first made it, he was concerned about if I could use it or not since my jaw would not seem to budge at all. It takes time. But let me tell you, when it works, there is nothing sweeter. Except being able to sleep with out any intervention.

Go slow and stay in your comfort zone. Most pain is muscular, ask your doc to show you spray and stretch exercises or possibly get you an aqualizer to relax muscles. Docs who do neuromuscular treatment understand pain and its treatment. Look for fellow or master of ICCMO.
