C-Flex is a patented new Respironics' technology that delivers a totally unique, more comfortable positive pressure therapy experience for the user. C-Flex tracks an individual's breathing patterns to provide personalized pressure relief when the user exhales (breathes out).
This helps to minimize air leaks around the mask and makes it easier for the patient to sleep through the night and be more compliant in using his or her therapy. Better compliance results in improved sleep and, ultimately, a better quality of life.
With us tonight to answer your questions is Respironics' Jay Vreeland, Global Product Manager for Sleep Therapy Devices & Humidification. During his tenure at Respironics, Jay has led the development of the new REMstar Family of devices including the REMstar Pro with C-Flex, REMstar Auto, and REMstar Plus.
In addition, he also led the development of the highly successful Encore Pro software and the BiPAP Pro with Bi-Flex. Jay holds a Bachelor of Arts from the University of Pittsburgh and a MBA in Marketing Management from Carnegie Mellon University.

C-Flex is a revolutionary new therapy to treat Obstructive Sleep Apnea. C-Flex tracks your breathing to lower the amount of pressure delivered when you exhale. This makes the therapy feel much more comfortable, or natural than traditional or standard CPAP therapy. The pressure relief is determined by two parameters: 1.) The patients' expiratory flow (how hard you exhale) and 2.) The C-Flex comfort setting, 1, 2, or 3. 1 provides the least amount of relief, while 3 provides the most. C-Flex provides variable pressure relief on each and every breath throughout the night.
Will I need a new prescription?
You do not need a new prescription to use the REMstar Pro with C-Flex. A CPAP prescription of 10 cms. will still be 10 cms on C-Flex. The REMstar Pro with C-Flex delivers your prescribed pressure and provides the same therapeutic benefit as CPAP.
How is REMstar Pro with C-Flex different from CPAP?
C-Flex therapy actually tracks your breathing on a breath to breath basis to lower the amount of pressure delivered at exhalation. You also have the ability to select from 3 C-Flex settings choosing the level of pressure relief that's right for you. C-Flex provides pressure relief at the beginning of exhalation then quickly returns to set pressure. CPAP provides the same constant pressure throughout the night during both inhalation and exhalation.
Is the C-flex part of the machine? Or the mask?
C-Flex is in the REMStar Pro device. C-Flex works to lower the amount of pressure delivered at exhalation.
What is the difference between the C-Flex settings?
There are 3 patient adjustable C-Flex comfort settings. 1 provides the least amount of relief while 3 provides the greatest. Giving patients a range of adjustability seems to help them quickly find a level that works best for them.
I wake up periodically throughout the night, by morning I do not feel rested; can C-Flex help fix this problem?
C-Flex has helped some patients improve or decrease their sleep fragmentation. If a patient may have woken up 7-8 times during a night on CPAP, with C-Flex, they may only wake up 1 or 2 times during the night.
I am experiencing mask leaks; how can C-Flex help?
A typical CPAP patient experiences a slight "spike" in pressure when they exhale. This happens when a patients' expiratory flow collides with the CPAP pressure causing a slight "spike" or increase in pressure. This is typically when mask leaks occur, the excess pressure causes the mask to leak. C-Flex eliminates this phenomenon by lowering pressure at the point of exhalation. This again eliminates the sensation of increasing mask pressure while on CPAP.
I go to sleep at night and when I wake up my mask is off; how will C-Flex help with this problem?
Because C-Flex tends to increase periods of sleep and decrease sleep fragmentation, we have had a lot of patients say that until they have used C-Flex, they have never woken up with their mask on their face.
I am not feeling well but I use my CPAP every day; what might be the problem?
You still be experiencing sleep fragmentation, meaning that you may be waking up periodically throughout the night, when this happens you may never get to REM sleep where you body benefits the most. Or you may only be using your device 4 hours or less a night. A lot of patients take their mask off at some point during the night, which limits the hours of usage with their machine. Many patients never wake up with their mask on their face. Recent clinical studies have shown that 4 hours of use with a CPAP device a night is not enough to achieve the full benefit of the therapy.
The SmartCard is inserted into the side of the unit and records the date, time and therapy usage information. This card enables your clinician, physician or homecare provider to track your compliance and usage patterns. This simple effective card allows you to provide the necessary information to track your progress by simply mailing or brining in your SmartCard to share with your physician or homecare provider. This eliminates having to carry your machine around to have someone check the data on the machine. SmartCard technology also features the onboard FOSQ which stands for Functional Outcomes of Sleep Questionnaire. FOSQ is a questionnaire designed specifically for people with sleep disorders. The results allow health care professionals to see how therapy has improved the quality of your life. The answers to the FOSQ are recorded on the SmartCard located in the side of the C-Flex unit. This information provides valuable information about the effectiveness of your treatment.
The SmartCard in my machine is a different color; will it still work?
Yes, the previous version of SmartCard will work with the updated version of Encore Pro software.
FOSQ stands for Functional Outcomes of Sleep Questionnaire. FOSQ is a questionnaire designed specifically for people with sleep disorders. The results allow health care professionals to see how therapy has improved the quality of your life. The answers to the FOSQ are recorded on the SmartCard located in the side of the C-Flex unit. This information provides valuable information about the effectiveness of your treatment.
Encore Pro Data Management Software is the software used by your Homecare Provider or Physician to download the data from your SmartCard.
The SmartCard provides valuable information about the effectiveness of your treatment. The SmartCard is inserted into the side of the unit and records the date, time and therapy use information. This card enables your provider to deliver truly tailored therapy to you the patient.
Can I purchase the Encore Software so I can track my own compliance?
The Encore Software is a tool created for the Provider to help your physician track patient compliance. The software is not for sale to the public.
Can I buy the REMstar Pro with C-Flex directly from you?
No, because U.S. federal law restricts these devices to sale by or on the order of a physician, Respironics does not sell direct. We go through Homecare providers to distribute our Products to you the patient. The Homecare Provider is better able to work with you to help you with your equipment and explain how to use your new therapy.
Is there something I can take to my physician to show him/her so I can get the C-Flex machine?
Yes, we have marketing literature that you can take to the physician. To obtain literature or other information, you may go to www.respironicsremstar.com.
How can I try out the new C-Flex unit?
Contact your physician for further details. Be prepared to explain problems or concerns you are having with your current CPAP therapy.
Can I use my friends CPAP machine to see if I like the machine?
CPAP is available from a physician by prescription only.
Does C-Flex provide the same therapy as CPAP?
C-Flex provides the same therapeutic benefit as CPAP.
Do you need a higher or lower C-Flex pressure compared to your CPAP pressure setting?
No, C-Flex and CPAP pressures are equivalent.
How many hours a night should I be sleeping with C-Flex?

How does C-Flex responds when there is a mask leak?
The REMstar Pro with C-Flex provides compensation for most mask leaks and will adjust the pressure to compensate.
I have heard that C-Flex is only for people with pressure requirements greater than 10 cms., is that true?
C-Flex is not designed for a specific pressure range. C-Flex is available on pressures from 4-20 cms. Patients will benefit from C-Flex regardless of their pressure.
How do I know if my machine has C-Flex?
There is a blue sticker in the top of the unit enclosure stating C-Flex feel the difference.
Does C-Flex cause any side affects?
C-Flex delivers the same therapy as CPAP. Refer to the user manual for intended use and contraindications.
How can you lower a patient's pressure, won't they have apneas?
C-Flex only provides pressure relief at the beginning of exhalation, and then quickly returns to set pressure. Apneas occur at the end of the exhalation cycle, by then C-Flex is already back to set pressure to ensure the pneumatic splint necessary for the patient.
Can I set the C-Flex level myself? At what level should I start (1-3)?
You can access the user menu to allow you to change certain settings including the C-Flex setting. Follow the instructions provided with the machine to make any setting changes. Try each setting until you find a setting that is the most comfortable.
What does flow-based Smart CPAP mean? Is that the same as C-Flex?
Flow based Smart CPAP analyzes flow patterns as you breathe to see if changing the pressure will benefit you. Changes in flow patterns may occur before breathing events disrupt your sleep. Flow based Smart CPAP are Auto-CPAP devices. The REMstar Pro with C-Flex is not an Auto-CPAP device.
How will my doctor know if I need a C-Flex machine?
If your physician is not yet familiar with C-Flex, we would suggest that you take information to the physician with you. Information on C-Flex is available at www.respironicsremstar.com
I like the C-Flex, but I am still having problems upon exhalation. How do I get a BiPAP Pro?
You will need to contact your Physician to discuss treatment options however, we have a similar technology in our BiPAP Pro. It is called Bi-Flex, and it also is designed to improve the comfort of the therapy being delivered.
How do I get the Heated Humidification with these units? Do they come in the box together?
Heated humidification is generally sold separately. You need to mention to your physician or homecare provider that you want to be set-up with heated humidification.
Can I use C-Flex without a humidifier?
Yes, REMstar Pro with C-Flex can be used with or without a humidifier.
Can I get replacement parts for the Heated Humidifier?
The water chamber and rubber connector are available replacement parts.
I have a C-Flex with the Passover humidifier; how do I get the heated humidifier?
Contact your Homecare Provider and request heated humidification.
I have a C-Flex but I can't remember how to change the humidifier settings. What is the process?
Hold the humidifier button down until you hear 2 beeps. The last setting will appear on the LCD screen use the humidifier (up) or ramp button (down) to change the setting. The setting range is from 1-5.
Can my doctor prescribe the REMstar Pro with C-FLex without a Sleep Study?
Diagnostic and prescription decisions are made by your physician. Your physician would have to make a determination whether or not to do a sleep study.
How do I know if I need one of these machines?

Continuous Positive Airway Pressure (CPAP - pronounced "See-PAP") is the most accepted and effective treatment for OSA. CPAP provides airflow to the patient via a nasal mask. The airflow holds, or "splints," the airway open so air flows freely to the lungs.
I have a Respironics machine - provided by a local DME Am I able to switch for the new machine without a cost?
You would need to contact your Homecare Provider and ask about their procedure for replacing or issuing new machines.
How much do these machines cost?
Contact your Homecare Provider for the cost of the unit.
Does my insurance cover these machines?
Typically this is a covered expense by your insurance company. However, insurance coverage varies and you would need to check with your carrier to determine coverage.
Why don't all the CPAP unit's have C-Flex?
C-Flex is only available in the REMStar Pro with C-Flex.
How do I know which unit to ask for from my doctor?
Take information on the REMstar Pro with C-Flex with you when you see your sleep physician. Your physician will determine which unit is most appropriate to treat your condition.
I had my sleep study and was prescribed a CPAP. How do I order a C-Flex machine?
Let your physician know you want a REMstar Pro with C-Flex. Bring information with you to the appointment. You can also request a REMstar Pro with C-Flex from the Homecare Provider.
My machine is an older model. How do I get this new C-Flex machine?
Depending on your insurance coverage, machines can generally be replaced every 5 years. Contact your Homecare Provider for a new unit. You may have the option to purchase the unit yourself.
I have a REMstar Pro now but mine does not have C-Flex. Can I get C-Flex added?
To get C-flex you need to purchase an updated version of the REMstar Pro.
How is C-Flex different from Auto-CPAP?
Our REMstar Auto changes pressure to prevent throughout the night searching for the optimal level or pressure to reduce breathing events and ensure you receive a good night's sleep. C-Flex provides variable pressure relief, on a breath to breath basis, throughout the night, but only at the beginning of exhalation. It then returns to baseline or set pressure. An Auto-CPAP may adjust from 4 to 20 cms of pressure throughout the night, while a REMstar Pro with C-Flex is set to a prescribed pressure, and then provides pressure relief from the set, or baseline pressure.
What is the difference between C-Flex and ramp?
The ramp feature allows therapy pressure to gradually increase when you start to use the therapy each night. Ramp was designed to help you initially fall asleep when tuning on your CPAP. Ramp typically is prescribed for less than 45 minutes. C-Flex tracks your breathing on a breath to breath basis on each and every breath throughout the night to briefly lower the amount of pressure delivered at exhalation.
Do I still need to use my ramp feature?
As long as ramp has been prescribed it is up to you and your level of comfort.
Tell me more about the Remstar Pro with C-Flex altitude adjustment.
The unit automatically adjusts for changes in altitudes.
C-Flex is not the same as BiPAP. BiPAP has two different prescribed pressure settings one during inhalation (IPAP) and a lower setting during exhalation (EPAP). With CPAP, the pressure stays at a single level. BiPAP offers an alternative therapy for patients having significant respiratory problems who require additional ventilatory assistance or those having significant difficulties adjusting to CPAP. C-Flex is not BiPAP.
ill BiPAP have the C-Flex feature in the future?
BiPAP has what is called Bi-Flex and is available within the BiPAP Pro unit. Just like C-Flex Bi-Flex briefly drops the pressure as you begin to exhale. There are 3 settings and the settings can be changed by the patient. IPAP differentiates the two modes - C-Flex and Bi-Flex.
How do I order new accessories for these machines?
To order accessories, please contact the Homecare Provider who rented/purchased your machine. Most providers attach a sticker to the machine with their name and telephone number.
What is the warranty on the units?
2 years limited warranty
Can I purchase REMstar Pro with C-Flex tubing, filters and other accessories directly form Respironics?
To purchase any Respironics devices or accessories, please contact your Homecare Provider.
What Respironics masks can be used with C-Flex?
C-Flex can be used with all Respironics masks designed to for use with CPAP. No special attachment is necessary.
Can I purchase masks directly from Respironics?
To purchase a Respironics mask, please contact your Homecare Provider.
My mask is causing discomfort is this caused by my pressure level on the unit?
Reasons for mask discomfort vary from patient to patient depending on what mask you are using and the area of discomfort. Please contact your Homecare Provider to help you rectify the mask problem.
What type of masks do you carry?
Respironics carries the ComfortSeries of masks. We have nasal masks, full face, and total face masks. The masks include, ComfortClassic, Comfort Select, Profile Lite, Spectrum Full Face and The Total Face Mask. (We can link to each mask product page on our web site)
All Respironics Masks and accessories are LATEX FREE.
My mask is old and I like the (new ComfortSeries) mask. How often can I get a new mask?
Generally, depending on your insurance coverage, you can get a replacement mask every 3 months to 6 months. Please check with your insurance carrier for details.
Can I use a Full Face mask with C-Flex?
All Respironics masks can be used with C-Flex.
Am I able to use all types of masks with C-Flex?
Respironics masks are recommended for use with Respironics CPAP units.
Can I use C-Flex with my current mask?
C-Flex can be used with any Respironics recommended mask.
Can I use the Respironics ComfortSelect with C-Flex?
All Respironics masks can be used with the REMstar Pro with C-Flex.

Can I still use my Oxygen with C-Flex?
You can use oxygen with the REMstar Pro with C-Flex, please refer to your patient user manual concerning adding oxygen to the system.
I was just diagnosed with Central Sleep Apnea; can I use the C-Flex?
Central Sleep Apnea occurs when the brain causes the person to stop breathing. Central events are not caused by an obstruction therefore; CPAP is not the desired therapy. Central Sleep Apnea is treated with BiPAP Synchrony therapy. Generally patients with CSA need pressure support and sometimes a back-up rate to properly treat their condition.
This means Respironics is the only company able to make or sell a certain product or therapy. Patent literally means an exclusive right or title.
Can I purchase the International version of REMstar Pro in the US?
Because of certain FDA requirements only the Domestic version is available in the US.
Can C-Flex go through airport X-Ray?
Airport x-ray machines will not cause any damage to the REMstar Pro unit.
I travel allot is the C-Flex unit heavy?
The unit weights less than 4 lbs.
I didn't get a carry case with my C-Flex; can I order one?
There is a carry case available for the C-flex, Contact your Homecare Provider for ordering information.
The mask alert is a patient controlled feature which alarms when a large leak is detected in the circuit; for example the mask coming off.
The automatic on/off is a patient controlled feature which turns the unit off after 45 seconds with the mask off. An alert tone will sound and the unit will shut off. To turn the unit on with this feature just start breathing on the mask and the unit will automatically come on.
Can I change my pressure setting?
The pressure can be changed if prescribed by a physician. Contact your provider concerning pressure changes.
Can I use tap water in the REMstar Pro Heated Humidifier chamber?
Respironics recommends using distilled water in the Heated Humidifier chamber.
What will happen if I accidentally get water in the REMstar Pro?
Discontinue use. The unit is designed to take some ingress of water. There is some information in our manual to address this issue. Dependent on the severity of the situation, the unit may have to be sent in for service. Contact your Homecare provider or Respironics for directions on having the unit sent in for service.
The only cleaning that needs to be done is the grey pollen filter along with your tubing a mask. You can wipe the outside of the REMstar Pro with C-Flex with a damp cloth. DO NOT submerge the unit in water.
Are there maintenance requirements of the machine?
1) The gray pollen filter should be cleaned at least once every two weeks under normal usage and replaced with anew one every six months. The white Ultra-Fine filter is disposable and should be replaced after 30 nights of use or sooner if it appears dirty.
2) Clean the mask and tubing daily.
Can I use the C-Flex overseas and what do I need to do?
Yes, the REMstar Pro with C-Flex operates from 100-240 AC, if you are traveling to a country with a line voltage different than the one you are currently using, an international plug adapter is required to make your power cord compatible with the power outlets of the country to which you are traveling. You do not need an inverter or converter. The REMstar Pro with C-Flex automatically converts to the power/voltage you need. Again, you only need a plug adapter to plug your power cord into the wall outlet.
Can I use these machines with I go camping? What do I need to do?
Yes, we offer a DC cord solution (DC Power Cord System part number 1007979) for patients who like to go camping, boating etc. You can use the REMstar Pro with C-Flex in a stationary recreational vehicle, boat or motor home with a 12 volt DC power source.
What do I do if my CPAP stops working and I am away from home?
I would suggest that before leaving, contact your Homecare Provider and ask for a referral to another provider in the state where you will be traveling. If you are traveling overseas, you may contact Respironics for a listing of our international providers.
What is the address in Hong Kong or Japan (anywhere overseas) to purchase C-Flex?
